Role of  Community Health Workers

 The role of  community health -workers is crucial for early case detection and complete treatment  { EDCT } of malaria cases .  Chw are the front-line health workers who collected B.S. from house to house  visits  or perform RDT  to fever cases and gave timely radical treatment . They maintain records  of all blood smear  collection , RDT results , which are help him for data collection and reporting .

 Chw’s used the following register for VBD serveillance .

 Case detection and managment forms :- are   M-1 , M-2 , M-3 , M-4 .

1) M-1 :-  family health register  ie report of serveillance by chw/mpw /anm /asha .  It is actually a list of  all fever cases.This form is to be filled by any health workers  ,who is directly involved in case detection and treatment .

2)M-2 :- Laboratory request form  for slide examination  . It is filled in duplicate copies  by chw ,anm , asha  whenever blood slides need to be sent to the laboratory  for blood sample examination .If two B.S. collected  by health workers ina day and are needed to be examined  ,they are entered into M-2  and send  to the PHC lab. within two days , due to avoiding timelag or EDCT .

3 )  M-3 :- Record of slides examination in PHC Laboratory .

4 ) M-4 :-  Fortnightly reporting form  from subcentre ,Phc , district ,state  .It is villagewise ,subcenterwise , Agencywise  , fortnight  and monthly  consolidation  of all M-1 forms ,including  chw ,anm , asha , ( all total  active , passive , contact, mass  blood  smear Collection )  and  positive cases  details report . This report is made in triplicate and two copies are forwarded  to phc on 25 ‘th of month for the second fortnight and 10’ th of the  following month for first fortnight .

5 ) M-7 :- Details of malaria positive cases  register .

6) M-8 :- Blood smears receipt / entry register .

7) M-9 :- Epidemiological evaluation register or Master file .

8 ) M-11 :- Weekly / Monthly sevingram .

The other important register should be kept with onself  by the chw

  1. Container servey register
  2. Guppy fish hatchery register
  3. Guppy fish distribution log book
  4. Villagewise breeding places register
  5. Villagewise linelist of private -practioners and private lab register.
  6. Ten days working programme register ( Daily -Diary monthwise )
  7. Posirive -cases details register  for malaria ,filaria , Dengue , Chikun-guniya ,JE , CHK, Zika-virus and others )
  8. Migratory population servey register .
  9. MMDP -register .
  10. Formate -A , hydrocele patient register.
  11. Formate -B , Eliphantasiasis patient register.
  12. MF-6 :- Monthly report of malaria programme of phc spraying .

Integrated vector control forms

  1. VC:- Primary record of IRS
  2. VC-1S :- Wall Stencil
  3. VC-2 :- District IRS output form
  4. VC-3 :- Primary record of bednet delivery and impregnation
  5. VC-4 :- Bednet delivery and impregntion form.

Health workers  will take all precautions to use properly sterilised needles and clean slides while collecting blood smears.

He  will put stencil on the wall of each household with date and signature at the time of  fortnightly domiciliarly visits.

He will have a ten days  domiciliarly house to house visit schedule gsve by M.O. / DHO.

Role of  CHW  (Female )  ie ANM.

  1. ANM will be responsible for  collecting blood smears from all antenatal  and postnatal cases, under her care as well as from infants .
  2. During her visits ,if a pregnant woman has history of fever or has fever , she will collect from her ,thick and thin blood smear and keepm records in M-2 register .
  3. It should be kept in mind that malaria is a very serious complicate in pregnancy  and postpartum period leading to high mortality .
  4. She will take the blood smears to the nearest malaria clinic /phc laboratory  on the priority within 24  to  48 hours .
  5. She will obtain the result of blood smears and give radical treatment  to malaria positive  as per  species wise drug  schedule / guidelines .
